Pay starting at $30.83 - $32.37 / hour DOE

Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience.
The Care Coordinator is in a professional position providing intensive, community-based therapeutic services to children, youth and families with serious mental health needs. The Care Coordinator will utilize the Wraparound process to increase safety, stabilize placement and support individuals/families in meeting their identified needs by building a team of natural and formal supports. The Care Coordinator will participate as a clinical team member, learning and practicing Wraparound principles and processes. The Care Coordinator will participate in crisis management and the on-call rotation.

Provide direct helping interventions that:
Respond to crises in a prompt, effective and collaborative manner.
Provide and/or facilitate the provision of a range of therapeutic responses that support the overall care plan.
Utilize parent/family expertise in problem solving around specific needs.
Modify and adjust individual intervention techniques for each situation without changing the direction of the plan.
Partner with the family in order to assist them in utilizing the resources offered by formal and informal sources within their community.
Seek assistance and report incidents to the right people at the right time.
Facilitate others, including a team, to implement the care plan from beginning to successful outcome:
Communicate plan, strategies and role to team members, including involved CCS staff.
Consult with, recommend and provide training to team members around specific interventions.
Maintain a focus on strengths, needs and creative solutions and inspire others to follow this format in problem solving.
Reevaluate, modify, and redesign care plan based on new information.
