Position Summary:
Performs direct and indirect patient care activities under supervision of licensed personnel.
Establishes and maintains positive interpersonal relationships and promotes effective teamwork with patients, families, coworkers, and the community to meet customer service goals.
Performs direct patient care activities while maintaining maximum comfort and safety of patients.
Assists with admission, transfer, and discharge of patients.
Obtains and maintains accurate patient data and documents appropriately in medical records, including vital signs, intake and output, height and weight, etc.
Recognizes and communicates changes in patients vital signs, physical and/or mental condition to the Registered Nurse.
Provides personal care assistance with activities of daily living to patients, including:
• Bathing • Dressing • Eating • Drinking • Oral care • Elimination • Transportation
Assists Registered Nurse in skin care, including:
• Repositioning • Cleaning of wounds • Pressure ulcer prevention
Demonstrates ability to assist in patient transfers effectively and safely.
Responds appropriately to emergency situations or unplanned events.
Assists with falls prevention activities on the nursing area, responding quickly as appropriate to enhance patient safety.
Position Qualification:
High School diploma or GED required; Licensure as a Nursing Assistant in the state of Tennessee preferred.
