AuthoraCare Collective is currently seeking a Nursing Clinical Manager - Home-based Hospice Care. This position is full-time Monday - Friday 8:00am - 5:00pm supporting our Greensboro Branch and service areas. Must be currently licensed as a RN in North Carolina and in good standings with the NCBON. Management experience preferred.

Job Summary:

Has shared authority and responsibility for administration of nursing for Home Care patients in conjunction with the Senior Director of Home- based Hospice Care. Plans, organizes, and leads a cost-efficient standard of nursing care. Ensures professional standards and compliance requirements are met with local, state, and federal licensing and credentialing agencies. Shares responsibility for serving as a clinical resource to the clinical staff.

Shared oversight of employee competencies and performance appraisal. Conducts documentation and quality monitoring audits. Manages EMR system standard workflow, orders and tasks. Ensures accuracy of day to day operations within the EMR system. Shared oversight and accountability in experience of care scores, quality metrics, agency core values and behaviors. Ownership and timely follow-up of consumer complaints. Is accountable for nursing caseloads, assigning, and rebalancing, when necessary. Will facilitate and/or support weekly IDT meetings.

Education and Experience:

  • Graduate of an accredited School of Nursing

  • Must be currently licensed as an RN in North Carolina and in good standings with NCBON.

  • Three to five years' experience in health care, preferably in hospice.

  • Trained and experienced in areas of patient care, hospice philosophy, and knowledge of homecare and an understanding of care and services for which the position supervises.

  • CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

  • An in-depth knowledge of hospice, interdisciplinary team relationships, and community resources.

  • Excellent assessment, clinical nursing skills and effective interpersonal and communication skills.

  • Must have the ability to be innovative, to communicate effectively and to relate well with others.

  • Possess good organizational skills and demonstrates the ability to supervise and manage effectively.

  • CPR Certification.

  • Bachelor's degree in nursing preferred.

  • Three to five years management experience in health care, preferable hospice preferred.

  • CHPN Certified preferred.

  • Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Key Responsibilities:

Manages Nursing Care Services

  • Shares 24-hour accountability for nursing services.
  • Provides clinical supervision.
  • Responsible for ensuring daily coordination of patient care and continued assessment of patient needs.
  • Routinely evaluates nursing services and makes recommendations relative to changes in nursing departments.
  • Supports the development and revisions of the plans of care through Team Conference and other formal and informal planning activities.
  • Prepares and reviews reports.
  • Provides in-services and clinical care training.
  • Provides ongoing monitoring for compliance with internal initiatives.
  • Plans coverage for self and nursing staff when unavailable to work.
  • Coordinate's nursing assignments based on patient need and plan of care.
  • Recommends modifications of existing or development of new clinical policies and procedures based on practice patterns and needs.
  • Supports and monitors pharmacy benefit manager, supplies as well as DME portal as needed.
  • Ensures nursing documentation per agency standards and professional standards and compliance requirements are met with local, state, and federal licensing and credentialing agencies. Serves as clinical resource for staff.
  • Arrange or assist with direct care during emergencies or staffing shortage.

Staff Supervision

  • Provides supervision and evaluation of clinical staff to include nurses.
  • Makes visits with staff to assess competencies and provision of services.
  • Coaches and advises staff to facilitate accomplishment of agency's mission by maintaining an environment that fosters mutual support, learning, and professional growth.
  • Follows up with concerns noted on chart audits, grievances, and incident reports with patients, families and/or relevant staff as well as provides employee coaching as needed.

Quality Management

  • Support department performance improvement process.
  • May facilitate the identification of needed areas of improvement, planning and implementation of improvements, evaluation of outcomes, further necessary improvements, and ongoing performance improvement process.
  • Follows up with concerns noted on chart audits, grievances, and incident reports with patients, families and/or relevant staff.
  • Serves on Performance Improvement Work Groups.
  • Support activities to achieve patient/caregiver satisfaction goals.

Financial Accountability

  • Routinely monitors line items, maintaining expenses at or below budget and/or maintaining a relationship between costs and service volume.
  • Makes recommendations and/or adjustments which favorably impact operating performance.
  • Responsibly manages items under one's span of control and supports other managers and directors in doing likewise.
