Schedule: 5 days/week, 7.5 hrs/day, 37.5 hrs/week, No On-Call
Schedule Notes: Must have 2 years of discharge planning and utilization review experience. CCM certification required.
Work Setting: Care Management / Transitional Care
Pay Range: $93-$98/hr.
Required Skills & Experience:
- Seven (7) years of nursing experience required.
- Five (5) years of supervisory or leadership experience required.
- Care Management experience.
- Discharge Planning experience.
- Utilization Review experience.
- Program development and performance improvement experience.
- Experience supervising multidisciplinary care management teams.
Education:
- Bachelor's Degree in Nursing from an accredited program.
Preferred Education:
- Master's Degree in Nursing.
Required Certifications & Licensure:
- Active RN License.
- Primary Source Verification of RN License from NYSED.
- BLS Certification.
- CCM Certification.
Job Summary:
Under the general supervision of the Director of Nursing, assists with the planning, organization, direction, coordination, and evaluation of nursing services and educational programs within the Transitional Care Division of Care Management. The role focuses on reducing 30-day readmissions, supporting high-risk patients following discharge from inpatient medical-surgical units and the Emergency Department, and ensuring successful transitions back into the community.
Duties & Responsibilities:
- Assist in planning, organizing, directing, coordinating, and evaluating Transitional Care programs.
- Perform duties of the Director of Nursing in their absence.
- Develop policies, standards, and educational initiatives related to Care Management and Transitional Care.
- Provide oversight and direction to Accountable Care Managers (RNs), Care Coordinators, clerical staff, and ancillary personnel.
- Monitor patient care performance and collaborate with Nurse Managers on corrective action plans.
- Develop and implement programs to improve patient outcomes and employee performance.
- Support initiatives focused on reducing hospital readmissions and improving transitions of care.
- Organize and oversee care coordination activities for high-risk patient populations.
- Develop and maintain systems for tracking medication errors, counseling, and quality metrics.
- Ensure optimal coordination of care across the continuum to support safe community reintegration.
- Participate in quality improvement, performance monitoring, and service enhancement initiatives.
