Community Care Physicians CCP, the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Community Care Family Practice Colonie, a well-established primary care practice providing comprehensive care to patients of all ages.
Position Overview
As an Advanced Practitioner at Community Care Family Practice Colonie, you will deliver full-spectrum family medicine services in a collaborative team-based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling flexibility designed to promote work-life balance and long-term professional satisfaction.
Responsibilities
• Conduct comprehensive patient evaluations and manage acute and chronic conditions. • Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed. • Provide preventive care, health maintenance, and patient education. • Collaborate closely with physicians, nurses, behavioral health consultants, and support staff to ensure continuity of care. • Maintain accurate, timely clinical documentation in accordance with organizational standards.
