Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital. POSITION SUMMARY:Community Health is seeking a Nurse Practitioner to work at our Castleton Express clinic. Candidates are expected to be knowledgeable and skilled in the delivery of express care. Responsibilities include, but are not limited to: • Obtains complete medical history and physical examination on patients.• Reviews medical records.• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.• Establishes presumptive diagnosis.• Orders appropriate laboratory and diagnostic procedures.• Develops a therapeutic plan for patient.• Sutures wounds.• Prepares written prescription orders for drugs and controlled substances.• Instructs patients and family regarding medications and treatment instructions.• Provides recommendations regarding health and illness prevention.• Recommend community resource to meet patient/family needs.• Performs independent evaluations and treatment procedures in emergency situations.• Provides monitoring and continuity of care between physician visits.KNOWLEDGE, EDUCATION AND SKILLREQUIREMENTS:3-5 years Express / Urgent Care Practice Experience.Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.Strong inpatient skill set.Quality-focused team player.Excellent organizational, time management and problem-solving skills.Ability to function effectively as part of a multi-disciplinary team.Effective oral and written communication skills.Commitment to providing exceptional patient care.BENEFITS:Community Health seeks to offer our Providers a very robust benefits package:Market leading compensation - commensurate with experience.Generous bonus opportunity.Up to $10,000 in relocation expenses.Medical, dental, and vision insurance.Retirement plan - 403(b) with employer match.Paid malpractice insurance.Generous leave time: up to 4 weeks plus holidays.Work-life balance with 4-day work weeks.On-Call 3 weeks per year on average.CME leave (1 week/year) and CME allowance ($6,000/year)Opportunities to apply for school loan repayment, up to $20,000 a year.#CHJOBS
