Nurse Practitioner | PRN | Hospice

Position Summary

The NursePractitioner is a registeredprofessional nurse with advancedgraduate educationand clinical training whoprovides comprehensive hospice services in compliance with the state NursePractice Act and under the supervision of a collaboratingphysician. He/Sheprovides hospice care through theevaluationandassessment of thehealth history, health promotion, diagnosing medical conditions, management of healthproblems by directingand developing the plan of care, prescribing medications or treatments, coordinatingand collaborating withpatients, families and other healthcarepractitioners. Under the supervisionof a physician, the nurse practitioner visits patients, completes physical assessments, performs face to faceassessmentsandevaluates hospiceeligibility as assigned in consultation with thehospice interdisciplinary team. The nurse practitioner builds from the resources of the community toplan and direct services to meet theneed of individuals and families within their homes and communities. 

Key Responsibilities

Patient Care

• Provides medical care, palliates and manages terminal illness in compliance with the state NursePractice Act under the supervision of a collaboratingphysician. 
• Completes history andphysical examinations, by evaluating the patient's medical conditionand healthhistory, ordering and interpreting results from laboratory anddiagnostic tests or  procedures, diagnosing health conditions anddocuments the findings in thepatient's 
• medical record. 
• Manages health conditions and terminal illnesses by developingan individualizedplan of 
• care, prescribing medications and/or treatments, obtaining consultations or making referralsand coordinatinghospice care services. 
• Completes the hospice face to face visit requirements prior to the third and subsequenthospicebenefit periods toassess hospiceeligibility based on the hospice conditions of  participationand the local coveragedeterminations. Provides clinical information to thehospice medical director for interpretation for eligibility for hospice recertification. 
• Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. 
• Reviews clinical information, coordinates andoversees patient care in consultation with thehospice interdisciplinary teamand other healthcarepractitioners. 
• Assures that patient receives appropriate measures to control symptoms through collaboration with interdisciplinary teammembers. 

• Reviews and develops protocols for treatment and proposes options for interventions basedon the literature in collaboration with the collaboratingphysician. 
• Consults the collaborating physician when thepatient's plan of care is outside standardizedpractice and protocols.    

Communication

• Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's planof care. 
• Consults with thepatient's attendingphysician, the hospicemedical director and other healthcarepractitioners regarding thepatient's eligibility for hospice care, ongoing careneeds and medical management related to thepatient's terminal condition. 
• Communicates withother community healthpractitioners to coordinate the planof care.
• Attends and participates inhospice interdisciplinary group meetings.
• Completes, maintains and submits accurateand relevant clinical notes, physical 
• examinationnotes, assessment visit notes, medical orders, collaboration notes and otherdocumentation in themedical record. 
• Educates/counsel's patients, families, and/or caregivers as topreventative care, medicalproblems, psychological problems, and spiritual problems in conjunction with the 
• interdisciplinary team tomeet the total needs of patients.   
• Provides andmaintains a safeenvironment for the patient.
• Assists the patient and family/caregiver andother team members inproviding continuity of care. 
• Works in cooperation with the family/caregiver and hospice interdisciplinary group to identify the goals of careand meet the careneeds of thepatient and family/caregiver. 

• Serves as anursing resource for consultation and educations to members of the
• interdisciplinary team and other healthcare practitioners in the community. 
• Establishes, builds and nurtures relationships with staff and community referral sources to facilitateprogramgrowth.    

Additional Duties

• Maintains knowledgeof and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services 

• Ensures compliance with the Medicare conditions of participationandother state regulationsgovern the provision of healthcare. 
• Complies withall Health InsurancePortability andAccountability Act (HIPAA) requirements inaccordance with federal, stateandorganizational policies. 
• Participates inorganizational monitoring of thequality of medical services andquality improvement initiatives. 
• Assumes responsibility for personal growth. Develops, maintains and upgrades professional knowledgeandpractice skills through attendanceat seminars, conferences and 
• participation in continuingeducation and in-service classes. 
• Fulfills the obligationof requested and/or acceptedassignments.
• Demonstrate knowledge in communication and counseling patient/family indealing withend-of-life issues.
