Job Overview:

We are seeking an experienced and compassionate Advanced Practice Registered Nurse (APRN) / Nurse Practitioner to join our Adult Medicine Clinic. The APRN will provide comprehensive primary care services to adult patients, focusing on health promotion, disease prevention, diagnosis, treatment, and ongoing management of acute and chronic medical conditions.

This role offers the opportunity to work collaboratively with physicians, nurses, and interdisciplinary healthcare professionals to deliver high quality, patient-centered care in an outpatient clinical setting.

Key Responsibilities:

  • Provide comprehensive primary care services to adult patients in an ambulatory clinic setting.
  • Conduct patient assessments, obtain detailed medical histories, perform physical examinations, and develop appropriate treatment plans.
  • Diagnose and manage acute and chronic health conditions according to established clinical guidelines and protocols.
  • Order, interpret, and evaluate diagnostic tests and laboratory results.
  • Prescribe medications and treatments in accordance with state regulations and clinical best practices.
  • Educate patients and families regarding disease prevention, health maintenance, wellness, and treatment plans.
  • Coordinate care with physicians, specialists, and other healthcare providers to ensure continuity of care.
  • Maintain accurate and timely documentation within the electronic medical record system.
  • Participate in quality improvement initiatives and support organizational goals for patient outcomes and satisfaction.
  • Adhere to all federal, state, and organizational policies, procedures, and regulatory requirements.

Qualifications:

  • 8 years of relevant working experience.
  • Graduate of an accredited school of nursing.
  • Possess valid State of Connecticut licensure.
  • Have or be eligible to obtain DEA and state narcotic registration.
  • Have a deep knowledge of nursing theory and practice or physician assistant theory and practice coupled with a record of providing high quality, comprehensive, professional clinical care to patients according to established standards, protocols and practices.
  • Have skill taking medical history, assessing medical conditions and interpreting findings.
  • Candidates with 1 year's experience working in an ambulatory setting or providing urgent.
  • Employment at CHS must successfully pass a pre-employment background check and drug screen.

Hours:

  • Full Time - 40 hours per week
  • Monday - Friday
