Position Overview
CLF Consultants LLC seeks a Mississippi-licensed Nurse Practitioner or Physician Assistant for a full-time, weekday, on-site clinical role at a state veterans nursing home in Jackson, Mississippi. This provider is the daily clinical presence in the building: the first call for the nursing staff, the familiar face for residents and families, and the engine of the facility's day-to-day medical care.
The role practices under a Board-approved collaborative protocol with a board-certified Medical Director who is on-site weekly and reachable at all times. The work is classic long-term-care medicine for a veteran population: admission workups, scheduled follow-up visits, acute change-in-condition evaluations, medication management with deliberate attention to polypharmacy and psychotropic stewardship, wound and falls follow-up, and honest, well-documented conversations with residents and families about goals of care.

Key Responsibilities
Conduct daily on-site clinical rounds Monday through Friday beginning at 7:00 a.m.
Complete admission assessments, medication reconciliation, and initial orders for newly admitted and readmitted residents
Perform scheduled periodic medical reviews and assist with annual comprehensive examinations on the resident panel
Evaluate acute changes in condition, initiate diagnostics and treatment within the Board-approved protocol, and escalate to the collaborating physician when indicated
Manage medication regimens with attention to polypharmacy reduction, gradual dose reduction documentation for psychotropic medications, and antibiotic stewardship
Review and act on laboratory and diagnostic results; document every encounter with signed, dated progress notes
Participate in interdisciplinary care planning, discharge planning, and family communication
Support facility quality improvement activities and in-service education alongside the Medical Director
Comply with facility infection prevention and infectious pathogen protocols
Required Qualifications
Current Mississippi license in good standing: Nurse Practitioner (Mississippi Board of Nursing) or Physician Assistant (Mississippi State Board of Medical Licensure)
National certification: AANP or ANCC for Nurse Practitioners; NCCPA for Physician Assistants
Eligibility to practice under a Board-approved collaborative protocol with prescriptive authority
Minimum 2 years of clinical experience in geriatrics, long-term care, internal medicine, family practice, or adult primary care
Current TB skin test and ability to pass a Mississippi State Department of Health background check before starting
Strongly Preferred Qualifications
Direct experience in skilled nursing, long-term care, or post-acute settings
Experience with dementia care, behavioral health in older adults, and end-of-life care planning
Experience caring for veteran populations or practicing in VA or state veterans home settings
Familiarity with CMS long-term-care documentation standards and state survey processes
Compensation and Engagement Details
$59 to $71 per hour (1099 independent contractor), commensurate with experience
Full-time, predictable weekday schedule: Monday through Friday, 8 hours per day, no weekends, no overnight call
