The Opportunity
This role offers the opportunity to work alongside Board-Certified PM&R physicians in a specialized rehabilitation environment where your clinical judgment and collaboration directly impact patient recovery outcomes. Rather than working in a high-volume primary care setting, you will focus on helping patients improve mobility, functionality, pain management, and overall rehabilitation progress.

What You'll Do
- Partner with PM&R physicians to evaluate, manage, and support patients in sub-acute rehabilitation and skilled nursing facilities
- Collaborate closely with therapy departments to help patients progress toward functional recovery goals
- Coordinate care with primary care providers, therapists, nursing staff, social workers, and family members to ensure continuity and quality of care
- Support on-site pain management and rehabilitation treatment plans designed to improve patient comfort and reduce hospital readmissions
- Maintain timely and accurate documentation while balancing efficient patient care delivery
- Contribute to a patient-centered, team-oriented culture focused on measurable rehabilitation outcomes and patient satisfaction
What Success Looks Like
Within the first 6-12 months, successful clinicians in this role will:
- Build strong working relationships with facility therapy teams, nursing staff, and collaborating physicians
- Help improve patient functional outcomes and rehabilitation progress across assigned facilities
- Contribute to reduced avoidable hospital readmissions through proactive patient management and coordinated care
- Maintain timely, accurate charting and documentation standards
- Become a trusted clinical resource for interdisciplinary rehabilitation planning and patient care coordination
What We're Looking For
- Licensed Nurse Practitioner or Physician Assistant
- Active NP or PA certification required
- Current Michigan licensure required
- Strong communication and interdisciplinary collaboration skills
- Ability to work independently while contributing to a team-based rehabilitation model
- Interest in rehabilitation, senior care, pain management, and functional recovery
- Reliable transportation for travel between skilled nursing facilities
