CASTLETONCLINIC:Community Health Castleton uniquely provides thecommunity with access to Primary Care, Behavioral Health, and Care Managementservices for the whole family. Primary Care services include on-siteBehavioral Health providers, Social Workers, and Care Management, along withpreventive programs like diabetes management and education. CastletonExpress Care, located within the same facility as Community Health Castleton,is open seven days a week, for the treatment of minor illnesses and injuries foradults and children.The town of Castleton is home to Lake Bomoseen andCastleton University. Picturesque, historic downtown Rutland, the thirdlargest city in Vermont, is the county seat and about 20 minutes fromCastleton. The Green Mountain National Forest, Killington, and Pico ski resortsare about a 45 minute drive from the quaint, small town of Castleton, and ashort drive to New York's Saratoga Springs and Lake George area.RUTLAND CLINIC:Community Health Rutland and Rutland Express Careserve the majority of those living in and around Rutland, the county seat andpopulation center of southern Vermont. The Rutland practice uniquely providesthe community with access to Primary Care, Behavioral Health and CareManagement services for the whole family. Primary Care services include on-siteBehavioral Health providers, Social Workers, and Care Management, along withpreventive programs like diabetes management and education. Rutland ExpressCare, located within the same facility as Community Health Rutland, is openseven days a week for the treatment of minor illnesses and injuries for adultsand children.Downtown Rutland, the third largest city in Vermont,is the county seat and home to Rutland Regional Medical Center. The GreenMountain National Forest, Killington, and Pico ski resorts are a short drivefrom the picturesque historic downtown area.POSITION SUMMARY:Community Health is seeking Nurse Practitioner or Physician Assistant to work at our Rutland, Castleton and Brandon clinics. Candidates are expected to be knowledgeable and skilled in the delivery of primary care.
