Overview:

Community Health Partners is seeking a full‑time Advanced Practice Provider (APPs) to join the Advanced Laparoscopic Surgery Associates (ALSA) team. In this role, the APP will provide comprehensive pre‑operative and post‑operative care for bariatric and advanced laparoscopic surgery patients, working closely with attending surgeons and the interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, performing medical diagnoses and perscribing appropriate therapies. The APP will support patients throughout the surgical continuum, ensure high standards of safety and quality.

Responsibilities:

In this role, you will:

 

  • Provides direct assessment, evaluation and primary patient care, complying with medical protocols and procedures.
  • Provides direct patient management in collaboration with physicians; participates in daily rounds, patient assessments and care plan development.
  • Orders, interprets and evaluates diagnostic tests to identify and assess patient's healthcare needs. Furnishes medications based upon written protocols.
  • Ensures that patients are counseled regarding treatment plans and the use, dosage and possible side effects of medications.
  • Makes referrals to appropriate providers when patients require subspecialty medical attention beyond the area's scope of service.
  • Documents and maintains precise and complete records of patients.
  • Distinguishes between normal and abnormal findings.
  • Provides education, training, information and consultation services to interdisciplinary team, patients, family and caregivers.
  • Performs other duties as assigned.

Qualifications:

Education and Experience Minimum Requirements:

 

  • Completion of a Nurse Practitioner or Physician Assistant program required
  • Credentialed by the CMC Medical Staff Office as an Allied Health Professional

Licenses and Certifications:

 

If qualifying as a Nurse Practitioner, the following is required

  • NP - Current State of California Nurse Practitioner License required
  • NPF - Nurse Practitioner Furnishing Number required


If qualifying as a Physician Assistant, the following is required

  • PA - State of California Physician Assistant License required
