Job Description
GENERAL OVERVIEW
This job is an integral part of the care delivery system, working together to provide a healing environment by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on-call coverage on a rotational basis as required.
CONTINUITY OF CARE PRIMARY CARE
Primary Care allows APPs to develop long-term relationships with patients managing their health over time and providing comprehensive ongoing care.
PREVENTATIVE HEALTH FOCUS
A significant aspect of primary care is preventative medicine, giving APPs the opportunity to educate patients, promote wellness, and proactively manage chronic conditions to improve long-term health outcomes.
BUILDING PATIENT RELATIONSHIPS
The nature of primary care fosters strong, trusting relationships with patients and their families, leading to a deeper understanding of their health journey and personal circumstances.
DIVERSE CLINICAL PRESENTATIONS
Primary care offers exposure to a vast array of common acute and chronic conditions, ensuring varied clinical experience and continuous learning.

Primary care is foundational to community health, and APPs contribute significantly to improving the overall well-being of the population they serve.

With the evolving landscape of healthcare, primary care settings often provide opportunities for APPs to specialize, take on leadership roles, or participate in quality improvement initiatives.
STABLE WORK ENVIRONMENT
Primary care practices generally offer a stable and predictable work schedule compared to more acute care settings.
ESSENTIAL RESPONSIBILITIES
Provides appropriate patient care, including recording patient medical history and physical assessment, developing therapeutic care plans, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation.
Communicates and coordinates patient care with patient families, community health services, and interdisciplinary teams, involving attending physicians regarding plans of care and involving physicians in clinical decision-making of any issues outside the CRNPs knowledge base or scope of practice.
Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physicians.
Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds, and provides wound care.
Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions, and recommends community resources as appropriate.
Triages patient telephone calls, provides monitoring and continuity of care, and participates in performance and/or quality improvement initiatives, ensuring compliance with all regulatory agencies.
May perform other duties, such as managing outpatient concerns and any other duty specific to specialty.
