GENERAL OVERVIEW

This job is an integral part of the care delivery system, working together to provide a healing environment by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on-call coverage on a rotational basis as required.

Will travel to various Express Care sites as needed.

ESSENTIAL RESPONSIBILITIES

Provides appropriate patient care to include:

• Recording patient medical history and physical assessment • Developing therapeutic care plan • Ordering diagnostic tests • Interpreting test results • Providing accurate medical record documentation

Communicates and coordinates patient care with:

• Patient family • Community health services • Interdisciplinary team

Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNPs knowledge base or scope of practice.

Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physicians.

Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds, and provides wound care.

Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions, and recommends community resources as appropriate.

Triages patient telephone calls, provides monitoring and continuity of care.

Participates in developing and implementing performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.

May perform other duties such as managing outpatient concerns and any other duty specific to specialty.

Other duties as assigned or requested.

QUALIFICATIONS

Required:

• Masters degree MSN from the practicing State Board of Nursing Approved Masters Program • National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP) • American Nurses Credentialing Center (ANCC) National Certification Corporation (NCC) or Pediatric Nursing Certification Board (PNCB) • Practicing-state licensure as a Nurse Practitioner • Prescriptive Authority from the practicing state within 60 days of hire • Drug Enforcement Agency Licensure within 60 days of hire • CPR - American Heart Association • Act 34 Criminal Background Clearance Certificate • Act 33 Child Abuse Clearance Certificate • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Preferred:

• Masters degree in Physician Assistant Studies • Current PA Physician Assistant License • PA-C or board eligible. Ongoing successful recertification NCCPA as a PA-C within 90 days of hire • Drug Enforcement Agency Licensure within 60 days of hire • CPR - American Heart Association • Act 33 Child Abuse Clearance Certificate • Act 34 Criminal Background Clearance Certificate • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Substitutions:

Four to ten years related experience and/or training or equivalent combination of education and experience.
