Compensation & Perks

  • Base Salary: $130K+ (flexible based on experience)
  • Bonus: Up to $10K annually

Schedule = Work-Life Balance
  • Monday-Friday, 8:00 AM - 5:00 PM
  • No nights. No weekends.
  • Limited call (only 3-4 weeks per year)
  • Call is triaged -- you're rarely contacted

Your Care Team Includes:

As a Nurse Practitioner, you'll deliver comprehensive primary care in a collaborative, team-based setting. You'll manage chronic conditions, guide patients through their care journey, and build meaningful long-term relationships.

Responsibilities include:
  • Patient assessments, exams & diagnostics
  • Chronic disease management (diabetes, COPD, CHF, dementia, etc.)
  • Developing personalized care plans
  • Patient & family education
  • Coordinating care with interdisciplinary team
  • Clinic-based care with some home visits & telehealth

What We're Looking For
  • Board-Certified NP (Adult, Adult-Gero, or Family)
  • Active NP license & DEA (or eligibility)
  • 2+ years of outpatient primary care experience required
  • Comfortable working independently
  • Experience with value-based care (preferred)
  • Athena EMR
  • chronic disease management, wound care, injections, ABIs, or imaging interpretation
