What if your next employer invested in your future instead of simply hiring you for the skills you already have?

Most healthcare organizations want providers who already possess every certification, every credential, and every specialty skill.

Twin Cities Physicians sees things differently.

We're looking for a Nurse Practitioner who wants to grow into a highly sought-after specialty while caring for one of the fastest-growing patient populations in healthcare.

If you already have wound care certification, fantastic.

If you don't, we'll help you get there.

Twin Cities Physicians will pay for your wound care certification, creating an opportunity to expand your clinical expertise while building a long-term career in senior medicine.

Our providers deliver care directly inside assisted living communities, memory care settings, skilled nursing facilities, and long-term care communities throughout Minnesota. As part of our growing team, you'll combine primary care, chronic disease management, community rounding, and wound care services to support patients where they live.

Organizations such as Bluestone Physician Services, Genevive, and Lifespark have helped demonstrate the growing demand for community-based medicine. Twin Cities Physicians continues to expand throughout Minnesota while investing in providers who want to continue developing professionally.

Why Providers Are Excited About This Opportunity

$120,000 to $160,000+ total compensation opportunity through a combination of base salary and productivity incentives.

Employer-paid wound care certification for qualified candidates who want to develop specialized expertise.

• Build valuable experience in geriatrics, chronic disease management, wound care, and community-based medicine.

• Join a provider-led organization serving senior communities throughout Minnesota.

• Receive mileage reimbursement, CME support, malpractice coverage, DEA reimbursement, PTO, and comprehensive benefits.

• Work with a patient population where your clinical knowledge and consistency can make a meaningful difference.

What You'll Be Doing

• Provide on-site primary care and rounding services throughout senior living communities.

• Manage chronic conditions, preventative care, medication reconciliation, diagnostics, and treatment planning.

• Deliver wound care services and participate in advanced wound management initiatives.

• Coordinate care with physicians, nursing staff, therapists, caregivers, and families.

• Support continuity of care while helping reduce avoidable hospitalizations and emergency room visits.

• Build expertise in both primary care and wound care management.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license.

• Experience in geriatrics, primary care, post-acute care, internal medicine, family medicine, or related settings preferred.

• Wound care certification welcomed but not required.

• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

• Passion for caring for aging adults and continuing to grow professionally.

The best investment in healthcare isn't always a building, a piece of equipment, or a new technology platform.

Sometimes it's a provider who wants to keep learning.

If you're looking for a full-time opportunity where your employer is willing to invest in your future while providing a strong compensation package and meaningful clinical work, we'd love to connect.
