Twin Cities Physicians is seeking a full-time Wound Care Nurse Practitioner to help solve the challenging cases that require deeper clinical assessment, stronger care coordination, and a higher level of attention.
Our providers serve assisted living communities, memory care settings, skilled nursing facilities, and long-term care environments throughout the Twin Cities Metro. Rather than treating issues from a distance, you'll be directly involved in helping patients overcome barriers to healing and recovery.
Organizations such as Bluestone Physician Services, Genevive, and Lifespark have demonstrated the growing need for specialized providers within senior communities. Twin Cities Physicians continues to expand by bringing advanced clinical resources directly to the patients and communities that need them most.

What You'll Be Doing
• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Identify barriers to healing and implement evidence-based interventions.
• Support continuity of care while helping improve clinical outcomes.
Qualifications
• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
