Twin Cities Physicians is seeking an Advanced Wound Care Nurse Practitioner to serve senior living communities throughout the Twin Cities Metro. This role combines advanced wound care, community rounding, chronic disease management, and senior-focused medicine into a practice environment that few providers ever experience.
You'll work directly within assisted living communities, memory care settings, skilled nursing facilities, and long-term care environments, helping patients receive specialized care where they live rather than requiring them to navigate multiple healthcare systems.
The demand for experienced providers in community-based senior medicine continues to grow, while opportunities to build a specialized practice like this remain relatively uncommon.
Organizations such as Bluestone Physician Services, Genevive, and Lifespark have demonstrated the need for this model. Twin Cities Physicians continues to expand throughout Minnesota as more communities seek greater access to on-site medical care.

What You'll Be Doing
• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help improve outcomes while reducing avoidable complications and hospitalizations.
Qualifications
• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and confidence managing medically complex patients.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
