Summary/Objective
Provides medical services within the scope of licensing, training, and credentialing, in accordance with specialty for patients of all ages, under the supervision of the Physician. Must meet credentialing requirements for NSHC, as applicable, and must demonstrate clinical competency in the area of Medicine.
Essential Functions
Assessment of Health Status
• Obtains a relevant health and medical history • Identifies medical health risks and needs • Updates and records changes in health status
Formulating Diagnosis and Implementing Treatment Plan
• Orders appropriate diagnostic tests • Determines treatment plan of care • Prescribes appropriate pharmacological agents • Prescribes non-pharmacological agents • Develops a client education plan • Makes appropriate referrals to other health professionals and community agencies
Maintaining Client Status
• Schedules return appointments for follow-up care, as necessary, according to best practice standards of care • Determines the effectiveness of the plan of care through documentation of client outcomes • Reassessment and modification of care plan as necessary to achieve goals
Promoting Maximal Health
• Provides educational opportunities for each patient • Incorporates patient's input in the development of the treatment plan • Provides information needed to make health care decisions related to appropriate utilization of health care resources
Responsibilities
Responds to tasks and completes charting in a timely manner to prevent any delay in care and promote coordination of care. Participates in quality assurance reviews, peer reviews, etc. on a periodic basis, including systematic review of records and treatment plans. Prepares reports and other documentation as required. Attends meetings and training sessions as required. Participates as a team member in the provision of medical and health care by collaborating with other care professionals to provide comprehensive care. Executes other verbal or written specific assigned tasks, requiring similar or lesser skills and abilities, some of which are continuing while others are occasional in nature.

Work Environment
Work is performed in an office environment. Involves frequent personal and telephone contact with patients, physicians, and other healthcare personnel. Work may be stressful at times. Interaction with others is constant and interruptive.
Travel
Travel outside of Northshore locations will be rare for this position.

Preferred Experience Requirements
Two years of experience working within a healthcare setting.
Minimum Education Requirements
Bachelor's degree in Nursing

Physical Requirements
While performing the duties of this job, the employee may be required to sit and/or stand for prolonged periods, work longer than eight (8) hour shifts, and to work both day/evening/weekend shifts. Work may include hand dexterity as well as the need to reach, climb, balance, stoop, kneel, crouch, talk, and hear. The employee must occasionally lift and/or move up to 50 lbs. While performing the responsibilities of the job, the employee is required to talk and hear.
