Job Description: Nurse Practitioner - Advance Practice (Part Time)

Division: Senior Services Status: Exempt Reports to: Director of Operations- Advance Practice Revision date: February 2025 Supervises: n/a

PURPOSE According to prescribed policies and procedures of the organization, including all applicable state, federal, and accreditation regulations, and under the general supervision of the Director of Operations- Advance Practice, the Nurse Practitioner is responsible for providing nursing care services to residents/clients as assigned, making observations on an advanced level, forming nursing judgments, carrying out nursing actions based upon understanding of the scientific principles involved, and reporting and recording observations appropriately. This position provides a part-time schedule, with work hours tailored to the departments needs.

ESSENTIAL FUNCTIONS

• Conducts physical exams and comprehensive medical and social history of individuals, including those who are healthy and those with acute illnesses and chronic disease. • Makes independent decisions regarding management and treatment of medical problems identified. • Orders, performs, and interprets laboratory tests for screening and for diagnosing. • Prescribes and orders appropriate diet, medications, and other forms of treatment such as physical therapy. • Performs therapeutic or corrective measures, including urgent care. • Refers individuals to appropriate specialist nurses or physicians or other health care providers. • Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, and medication use. • Evaluates the effectiveness of instruction and counseling and provides additional instruction and counseling as necessary. • Initiates and maintains follow-up of noncompliant patients. • Manages care of clients; develops plan of treatment and/or follow-up and monitors progress, determines when referral to another provider is necessary, makes necessary arrangements for further care, determines when hospital admission or emergency room visit is necessary, and determines when illness is resolved. • Assesses social/economic factors for each client, and tailors care to those factors. • Maintains familiarity with community resources and connects clients with appropriate resources. • Contracts with clients regarding provider responsibilities and client responsibilities. • Maintains an acceptable level of productivity to meet the facility goals. • Makes client home visits and provides care in the home as necessary. • Accepts after-hours calls and handles after-hours problems on a rotating schedule. • Participates in development of pertinent health education materials. • Participates in development of clinical practice guidelines. • Obtains informed consent from clients as appropriate and necessary. • Participates in community programs and health fairs, school programs, and workplace programs. • Represents the practice or the profession as an NP before local and state governing bodies, agencies, and private businesses as needed.

With respect to Resident Rights, ensures all care is provided with respect and dignity for the residents/clients, their families, and the staff. Reports all complaints made by residents/clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/resident property, and/or any other corporate compliance, drug-free workplace, safe work practices, all federal, state, and local regulations, and laws. Completes all annual education requirements timely, including but not limited to Compliance, Privacy, Security, and HIPAA training, and exhibits behavior as set forth by the Code of Conduct in the performance of their duties. Performs other duties as assigned.

JOB SPECIFICATION SHEET - Nurse Practitioner - Advance Practice (Part Time)

Education: Graduate of an accredited school of nursing with a Master's Degree in Nursing and a nurse practitioner certification.

Experience: Minimum of two years as an RN in a healthcare setting.

Travel: Moderate

Mental: Must have good communication, comprehension, and interpersonal skills. Must have the ability to speak, read, write, and understand English.

Skills: Must have working knowledge of Windows, Microsoft Office Suite, and World Wide Web.

Licensure: Current RN License and Nurse Practitioner in good standing for the state in which practicing.

VISION Normal: Consistent with standard workflow.

Consequences of Errors: High monetary responsibility.

Supervision Received: General supervision: Employee generally operates by himself/herself checking with supervisor when in doubt.

Working Conditions: Somewhat disagreeable; elements such as noise, dust, heat, and oil exist but not to the extent of being continuously disagreeable.
