QUALIFICATIONS
  • Graduate of an accredited Bachelor's Degree in Nursing (BSN) program.
  • Master's Degree from an accredited Nurse Practitioner program.
  • Board Certified Adult Nurse Practitioner (ANP-BC).
  • Minimum of five (5) years of outpatient experience managing Chronic Kidney Disease (CKD) Stage 3 and Stage 4.
  • Experience managing patients with uncontrolled hypertension.
  • Experience utilizing 24-hour blood pressure monitoring equipment and software.
  • Experience providing case management for post-kidney transplant patients.
  • Active, unrestricted Registered Nurse and Nurse Practitioner license.
  • Current Basic Life Support (BLS) certification.
  • Current or ability to obtain a valid DEA registration prior to providing services.
  • Strong communication, critical thinking, and interdisciplinary collaboration skills.
RESPONSIBILITIES
  • Provide comprehensive outpatient management of patients with Chronic Kidney Disease (CKD) Stage 3 and Stage 4.
  • Evaluate, diagnose, and manage patients with difficult-to-control hypertension.
    Manage and interpret 24-hour blood pressure monitoring equipment and associated software.
  • Provide ongoing case management for patients following kidney transplantation.
    Perform comprehensive patient assessments, develop treatment plans, and monitor patient outcomes.
  • Order, interpret, and evaluate diagnostic tests and laboratory studies.
  • Prescribe medications and treatments within the scope of practice and clinical privileges.
  • Collaborate with nephrologists, nurses, pharmacists, dietitians, social workers, and other healthcare professionals to coordinate patient care.
  • Educate patients and families regarding disease processes, medications, treatment plans, and lifestyle modifications.
  • Accurately document patient encounters using the electronic medical record while maintaining compliance with military treatment facility policies and standards.
  • Participate in quality improvement initiatives and maintain clinical competency in accordance with Walter Reed National Military Medical Center requirements.
