Job Summary and Qualifications

The Total Joint Program Nurse Navigator (TJNN) is responsible for overall coordinating and implementing the center's Total Joint Replacement Program. The TJNN will be guiding the total joint patient and their family through the program in all aspects of the care continuum from preoperative assessment and education through discharge and follow up. The TJNN will also complete an assessment of the patient’s current support system and their ability to return home after surgery. The TJNN will act as a liaison between all departments of the surgery center and advocate for the patient as needed.

What you will do in this role:

  • Responsible for the pre-operative assessment and evaluation of each potential total joint patient collaboratively with the ASC Medical Director.
  • Responsible for patient education including but not limited to 1:1 teaching and preoperative classes (both in-person and virtual education).
  • Utilizes and monitors a data tracking system for orthopedic outcomes through the care continuum such as patient satisfaction, surgical data, patient outcomes, and identifying areas that require improvement, to provide key statistics and outcomes at governance meetings.
  • Manage, monitor and assess current quality metrics required by national organizations and evidence-based practice for optimal clinical outcomes.
  • Facilitates quarterly total joint meetings with surgeons and multidisciplinary team members (if applicable).
  • Act as a point person and liaison during the patient’s stay at the center. Coordinates care issues with the surgeon and other members of the interdisciplinary team.
  • Complete post-op/discharge instructions with the patient and caregiver in conjunction with the surgery center’s recovery/PACU team.
  • Works in partnership with nursing to develop strategies to improve CHAPS scores to meet or exceed the surgery center’s established goals.
  • Works in conjunction with leadership and nursing staff to ensure competency and understanding the needs of the total joint patient.

Qualifications you will need:

  • 5 years applicable clinical experience.
  • Minimum 2 years in an acute care orthopedic setting, preferred.
  • Supervisory experience preferred.
  • Operational and strategic development experience preferred.
  • Associate Degree in Nursing or Nursing Diploma required.
  • BSN required. (internal candidates only - experience may substitute for degree)
