What Nurse Navigator contributes to Cardinal Health

Nursing is responsible for the delivery of safe and efficient quality nursing care. Examines patients and administers prescribed medications to facilitate healing and increase comfort. Nursing requires training and or licensed to care for patients.

The Nurse Navigator is a registered nurse who acts as patient advocates, guiding individuals through the healthcare system by coordinating appointments and educating them about their disease and treatment options. This position provides comprehensive care coordination for a disease-specific group of urology patients.

Responsibilities

  • Manage the case load of patients with chronic disease under the care of our physicians along with Medical Assistant
  • Serve as a focal point for the patients, directing them to the appropriate resources compassionately and in a timely manner
  • Assist patients with understanding the disease process, treatment options and through consultation planning, assist them with optimizing visits with their physician.
  • Ensure knowledge of resources and treatment options are consistently up to date
  • Improve healthcare access for patients and promote client knowledge
  • Collaborate with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
  • Consult with department staff to eliminate barriers to efficient delivery of care
  • Assess the patient's plan of care and develop, implement, monitor and document the utilization of resources and progress of the patient through their care
  • Serve as a liaison for referring physicians and assist with scheduling initial tests and consultations
  • Ensure that the patient and family understand the diagnostic processes, care strategy and recommended actions, and are provided with appropriate information in coordination with the physician
  • Respond to patient requests for information regarding the disease process, expected side effects and community resources
  • Establish and promote a collaborative relationship with physicians, payers, and other members of the health care team
  • Adheres to the laws and regulations of Medicare, Medicaid, and other governmental payers in relation to performance of job duties
  • Attends staff meetings, clinical meetings, and training in-services as scheduled and required
  • Adheres to the clinical dress code policy
  • Adheres to attendance guidelines
  • Promotes teamwork and works effectively with others. Displays a positive attitude at all times.
  • Demonstrates professionalism and compassion to patients and coworkers
  • Report in good faith to the unit manager or practice administrator any suspected violation of applicable laws and / or regulations
  • Understands and follows all HIPPA and Corporate Compliance guidelines to ensure the security of patient and corporate information
  • Responsible for in office dispensing duties that involve patient in the program
  • Performs other duties as assigned by manager or physician while maintaining a helpful attitude.
  • Strong oral and written communication skills for corresponding with physicians, patients and fellow employees.
  • All other duties as assigned

Qualifications

  • Associates Degree in Science
  • Bachelor's degree, preferred
  • Active Nursing license in Ohio, in good standing
  • Minimum 3 years RN experience, preferred
  • Effective communication and writing skills
  • Ability to multitask and work in a fast-paced environment
  • Urology experience, highly preferred
  • Oncology experience a plus
