Position Title: Nurse Manager
Department: Clinical
Reports to: Program Director
 
Fortcom Health Services is a mission-driven home care agency dedicated to improving lives through compassionate, community-based healthcare. We are currently seeking a Nurse Manager to oversee and support our  Adult Family Care (AFC) and Home Health Programs
 
This is a vital leadership role for a nurse who is passionate about clinical excellence, caregiver education, and helping medically fragile individuals receive high-quality care in their homes. 

 Position Summary
As the Nurse Manager, you will lead a team of Registered Nurses and oversee the clinical and administrative functions of our AFC and Home Health departments. You will be responsible for ensuring regulatory compliance, managing staff performance, and maintaining a high standard of care delivery. A key focus of this role is ensuring program efficiency, caregiver support, and member health outcomes. 

 Key Responsibilities:
  • Provide clinical supervision and leadership to all field-based RNs conducting home visits 
  • Oversee admissions and initial assessments, ensuring accurate completion of the MDS-HC and other required documentation 
  • Develop, review, and revise AFC Plans of Care to reflect member needs 
  • Collaborate with Program Directors to train, evaluate, and support caregivers 
  • Monitor member  health status and communicate significant changes to physicians 
  • Review caregiver logs and clinical notes to ensure ongoing compliance
  • Conduct orientation and ongoing training for caregivers related to health, safety, and aging
  • Assist with care coordination and transition planning for discharges
  • Submit timely semiannual health status reports to members’ Primary Care Physicians
  • Support field RNs by ensuring appropriate caseloads, timely documentation, and clinical guidance
  • Serve as a liaison between clinical and administrative departments 
  • Perform other related office duties as assigned by the Program Director or COO

Required Qualifications:
  • Bachelor’s or Master’s degree in Nursing (MSN preferred)
  • Current Massachusetts RN license in good standing
  • Minimum 5 years of nursing experience, with at least 2 in home health, AFC, or community-based care
  • Prior supervisory or Nurse Manager experience strongly preferred
  • Strong knowledge of MassHealth AFC regulations and documentation standards
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint) and EMR systems
  • Bilingual in Spanish
  • Must have reliable transportation and willingness to travel to member homes as needed

Compensation: $87,000.00 - $90,000.00 per year
