Essential Duties and Major responsibilities:

  • Organization & coordination of Suboxone and Vivitrol schedules, distribution of patients across sites and

provider time blocks. Management of provider registry for patient receiving MAT.

  • Be the patient contact and point person for accurate information for patients and providers regarding

protocols for Suboxone and Vivitrol patient education materials

  • Patient contact link for patient counseling, as well as patient contact link for primary care.
  • Provide support during screening, intake, induction, stabilization & maintenance phases of patient

treatment.

  • Conduct random nurse visits as directed by provider to insure compliance and minimize diversion of

prescribed medications.

  • Responsible for providing clear vision and oversight for all aspects of the program and any grants mandates

associated with the program.

  • Required to attend mandatory program meetings and trainings.
  • Ensure staff receive all appropriate training mandated by grants and/or CHC
  • Program oversight requires travel to, provider support, staff management and patient care at all CHC sites.
  • Develop and maintain relationship with DPH, including monthly submission of data management system file

extractions.

  • Develop individualized case management plan in coordination with the member and provider to reach timely

and appropriate patient centered goals.

  • Communication with providers, patients and the patient's families when appropriate
  • Maintains the treatment database for those patients enrolled in the program and pertinent to any associated

grants.

  • Monthly reporting to Quality Improvement Committee and other committees as requested.
  • Conduct audits and reviews to monitor compliance with agency policies and procedures by program staff.
  • Work collaboratively with CHC supervisors and department managers.
  • Participates in defining program standards, practices, and educational needs of OBOT/MAT staff
  • Ensures safe, compassionate, confidential
  • Annual evaluations, competencies, scheduling, delegating work and taking disciplinary actions when

needed for program staff

  • Counsels and support staff on personnel issues such as career development, work problems or

interpersonal conflicts.

  • Participates in quality improvement and PCMH initiatives.
  • Participates in Health Centers community activities (health fairs, OBOT community outreach)
  • Demonstrates excellent customer service and effective communication skills by email or calling via

telephone in regards to meetings, staff updates, and other correspondence as needed.

  • Demonstrates team building skills
  • Demonstrates analytic and data management skills using a variety of PC based software, including MS

Office (Word, Excel, and Power Point)

  • Demonstrates understanding and commitment to the mission of the health center
  • Demonstrates understanding and commitment to the established CHC Values and Standards
  • Works independently on special nonrecurring and ongoing projects as needed.
  • Perform related duties as required and assigned.

Minimum Qualifications:

  • Registered Nurse degree required, Bachelor of Science Nursing preferred
  • Work experience in an ambulatory medical office environment.
  • Proof of Leadership experience.
  • Currently registered as a Registered Nurse in the Commonwealth of Massachusetts and its Board of

Registration in Nursing

  • Computer skills for accurate data entry
  • AHA BLS Health Care Provider certification or equivalent
  • Demonstrated interpersonal relationship skills
  • Demonstrated written and verbal communication skills in English
  • Demonstrated ability to work in a fast-paced medical office environment

Supervision Exercised: Supervises OBOT/MAT program staff.

Competencies: Individual departments may develop competencies which will be communicated and maintained

through CHC's performance evaluation system.

Environmental Working Conditions: Usual working environment found in outpatient primary care office setting.

Exposure to blood products, body fluids, cleaning solutions, and medications.

Physical and Mental Requirements: The physical and mental demands are representative of those that must be

met by an employee to successfully perform the essential duties of this job with or without a reasonable

accommodation. The position requires the ability to self-transport, in a timely manner. The ability to travel out of

state to attend conferences or other health or health center related events.
