Compensation:
• The pay for this position is $44.14 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Highlights:
• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday-Friday, 8:00 AM – 5:00 PM EST
• Field and telephonic based role with approximately 50–75% travel throughout Wayne and Macomb Counties, MI
• Remote work from home when not conducting field visits and member care activities

Responsibilities:
• Conduct comprehensive assessments of member needs, eligibility, and healthcare goals through the use of clinical tools and data review
• Develop, implement, and coordinate individualized case management plans to support optimal health outcomes
• Evaluate member benefit plans and coordinate appropriate internal and external healthcare resources and services
• Utilize standardized case management guidelines, regulatory standards, policies, and procedures to ensure proper administration of benefits
• Perform member outreach, telephonic care management, and documentation in multiple systems
• Collaborate with healthcare providers, caregivers, and interdisciplinary teams to support continuity of care
• Maintain compliance with quality management processes, accreditation standards, and company policies
• Travel throughout assigned counties to conduct in-person member visits and community-based care management activities

Requirements:
• Active unrestricted RN license in the state of Michigan required
• Minimum of 3 years of clinical practice experience in hospital, home health, ambulatory care, or related healthcare settings required
• Case management experience preferred
• Managed care and/or healthcare industry experience preferred
• Strong computer skills including navigating multiple systems and keyboarding proficiency
• Excellent verbal and written communication skills
• Ability to multitask, prioritize, and adapt in a fast-paced environment
• Must possess a verifiable High School Diploma or GED

Education:
• Required: High School Diploma or GED
• Required: RN with current unrestricted Michigan state licensure
• Preferred: Case Management Certification (CCM)
