This field-based position will cover the following locations: Manhattan, Bronx, Brooklyn, Queens, and Nassau.
The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.
He/she will provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care.
He/she may work in a community, long-term care environment, or hospital.
He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team.
In all settings, he/she will provide in-person visits or telehealth visits, as appropriate.
Care will consistently comport with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation.
In the role as hospice NP, he/she will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care.
He/she will also contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed.
He/she will also help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage or for when young adolescents are incorporated into adult hospice teams.
In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures.
Other activities related to quality improvement projects may also be required, as appropriate.
Qualifications:
Master's degree in nursing and graduate of an approved Nurse Practitioner Program required
License/Certification: NYS Certified Nurse Practitioner (NP) required
Minimum of 1-year clinical experience as a NP provider preferred
Pediatric RN or NP experience strongly preferred
Prior experience working as a member of a specialist-level palliative care team or working in a hospice environment preferred
CPR-BLS
Valid NYS Drivers license
Hospice & Palliative Care certification strongly encouraged by year 2 of hire
Microsoft Word and Excel
Ability to work flexible hours to meet the needs of the position
