The Optum Care at Home program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of patients (beneficiaries) in their place of residence. The APC within Care at Home provides care to our highest-risk health plan beneficiaries and is part of an interdisciplinary team. Primary Responsibilities:

  • Program Management and Responsibilities:

  • Serve as a mentor and coach to APCs . The CSM will demonstrate clinical expertise and guidance, program insight, and support in all aspects of the role

  • Plan and conduct Ride Along visits in accordance with guidelines set forth by senior leadership. Travel is expected and is dependent upon specific market and geographical need

  • Assist in hiring and training new APCs

  • Ensure enterprise, department, HIPPA, and regulatory compliance: Maintaining security of Protected Health Information (PHI) and Intellectual Property (IP)

  • Responsible for all GSS/myHR functions: Update status change, terminations, PTO management, population and network development reconciliation

  • Perform yearly common reviews and continually manage performance. Implement coaching plans and/or corrective action plans related to standards set forth by business initiatives

  • Regular meetings and collaboration with supporting departments to ensure operational efficiency. Providing solutions to scheduling, assignments, and staffing plans

  • Plan and develop compelling market strategies and present to Senior Leadership during Business Reviews

  • Workforce management: Manages demands of a rapidly changing staffing environment with resilience. Analyzes and makes fact-based decisions utilizing forecasting model

  • Staff Development and Engagement

  • Host monthly team meetings to communicate updates on company goals and performance: Monitor and address field concerns and communicate to senior leadership

  • Represents Care at Home in a positive, solution focused manner that embodies our corporate values: Integrity, Compassion, Relationships, Innovation and Performance

  • Develop strategies to increase job satisfaction and reduce turnover rates Identify key talent and provide opportunities for professional growth and career advancement

Required Qualifications:

  • For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

  • For PAs: Current NCCPA certification and state licensure

  • Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations

  • Active and unrestricted driver"s license

  • In addition to a driver"s license, access to reliable transportation to complete home visits

  • Ability to navigate varied environments and to position oneself as needed to perform job duties

  • If you"re working in a state that allows NPs/PAs to practice independently (without Collaborative Agreement), you must get approval from your licensing board-if required. New hires who are eligible but haven"t applied yet must do so within 1 month of starting. If you"re not eligible at the time of hire, you must begin working toward eligibility within 1 month and apply for approval within 3 months of becoming eligible.

  • 5+ years of practice experience in a clinical setting

Preferred Qualifications:

  • 2+ years of management and leadership experience
  • Experience in a home setting
  • Experience in working within a managed care environment
  • Knowledge of HEDIS and STAR quality measures
  • Proficiency with electronic medical records and technology

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you"ll find a far-reaching choice of benefits and incentives. The salary for this role will range from $126,000 - $188,500 annually based on full-time employment. We comply with all minimum wage laws as applicable.

Fraudulent Activity Notice
