Position Summary


The Resident Care Coordinator provides daily clinical and operational oversight of resident care in Assisted Living and Memory Care. This role supports supervisors and care staff, coordinates new admissions, ensures regulatory compliance, and serves as a key point of communication between residents, families, and the clinical management team. Flexibility, leadership, and accountability are essential for success in this position.


Key Responsibilities


Resident Care & Clinical Oversight


  • Provide daily oversight of resident care, ADLs, and service delivery
  • Ensure care plans, assessments, and documentation are accurate and up to date in PCC
  • Monitor oxygen use, vital signs, weights, and other clinical indicators as ordered
  • Audit medication administration, ADLs, and care documentation for accuracy and compliance
  • Communicate with physicians and pharmacy as needed, especially in the absence of the Clinical Manager
Admissions & Transitions


  • Coordinate and manage new resident admissions
  • Ensure all admission paperwork is complete, scanned, uploaded, and properly filed
  • Verify baseline assessments, vitals, weights, and ADLs.
  • Set up residents in PCC and PalCare, including photos and pendants
  • Assign shower schedules and adjust services based on resident needs and family requests
Memory Care Oversight


  • Ensure memory care environments are safe, structured, and dementia‑appropriate
  • Monitor resident behaviors and support staff with appropriate care approaches
  • Ensure rooms, beds, and common areas are clean, uncluttered, and supportive of resident dignity
Staff Leadership & Support


  • Provide daily oversight and support to supervisors, med techs, and care staff
  • Assist with staffing coordination to ensure proper coverage based on census and acuity
  • Partner with leadership to address call‑offs, scheduling challenges, and overtime control
  • Recommend staff for promotion, med passer roles, and supervisory opportunities
Training & Compliance


  • Assist with orientation and ongoing clinical training for care staff
  • Support medication administration training under the direction of the Clinical Manager
  • Ensure readiness for audits, surveys, and regulatory compliance
  • Promote best practices, accountability, and a culture of teamwork
