We are seeking a dedicated LPN to provide high-quality care to adult and geriatric residents. In this role, you will deliver both direct and indirect nursing care under the supervision of a Registered Nurse, ensuring patients receive compassionate, age-appropriate treatment.
Key responsibilities include:
- Monitoring residents’ physical and psychosocial conditions and reporting findings to the care team
- Assisting in developing and updating individualized care plans
- Collaborating with nurses, physicians, and interdisciplinary team members
- Supporting a safe, healthy, and culturally sensitive care environment
- Participating in staff mentoring, training, and continuing education
- Providing skilled nursing care while maintaining clinical competency
This role involves working closely with residents and families to ensure quality outcomes and high satisfaction in a professional healthcare setting.
Location: Need to live 50+ miles from Waterloo, NY
Schedule:
- Full Time: 5x8s with Day/Evening/Night/Weekend rotations
- Part Time Evenings: 2 shifts week 1, 3 shifts week 2; Every Other Weekend (EOW) & Holiday required
- Per Diem: Minimum 24 hours (or 3 shifts) within a 4-week period, including at least 2 weekend shifts

Requirements
Active New York LPN License
BLS Certification (AHA or RQI only)
Minimum 1 year of LPN experience
Must be able to start on a Tuesday or Thursday for full-shift orientation
Experience or ability to care for adult and geriatric populations
Strong communication, teamwork, and clinical assessment skills
Willingness to work in an environment with potential exposure to bloodborne pathogens
