Job Title: Patient Navigator Location: Bronxville, NY Local, Remote or Hybrid: Local Schedule: Onsite; M-F 8:30-5PM Contract:17 Weeks Important Call Outs: NYP Vaya Coversheet Required Required Qualifications Education: HS Diploma/GED (R); Bachelor's (P) Experience: Patient registrar/navigation experience (R) Excellent oral and written communication skills (R) Diverse and culturally sensitive customer service skills (R) Ability to manage high stress, chronically ill population with complicated psycho-social issues (R) Experience in health care setting working directly with patients (P) Basic computer skills (Microsoft Word, Excel) (R) Bilingual (Spanish) (P) Certifications: N.A Technical Skills: N/A Other Requirements: N/A Key Responsibilities: JOB SUMMARY: The Patient Navigator functions as the primary care coordinator for patients and collaborates with key clinical staff to ensure comprehensive care and treatment. Coordinates appointments, provides logistical support, acts as a liaison between the patient and medical team, directs patient inquires to appropriate staff, and facilitates collection of medical records, insurance and financial information. Position includes direct contact and communication with patient, in person, on telephone or via e-mail. JOB DUTIES: Educates the patient on the purpose of the visit and schedules the appointment. Oversees the gathering of patient information and records from healthcare providers in preparation for appointment. Enters patient demographics including need for interpreter services into appropriate patient scheduling system. Contacts patient in advance to confirm appointment. Ensures that subsequent medical information is added to patient chart as necessary. Arranges for interpreter services for all appointments and treatments as needed. Tracks consultation, simulation and new appointments to be made available to patients as needed. Assists patient in waiting area with non-clinical aspects of paperwork. Coordinates multi-site appointments and schedules ancillary appointments as requested. Refers patient to appropriate staff regarding advance directives, community resources, transportation and insurance issues. Facilitates access to medical team and informs physician of status changes or obstacles to patient treatment compliance. Coordinates the scheduling of ancillary appointments as requested and follow up appointments, post treatment. Oversees the rescheduling of consultations and follow-up appointments due to physician cancellation. If working in the Oncology Program: Answers patient questions and provides resources on oncology cancer topics such as imaging, nutrition, chemotherapy effects etc. Provides referrals to internal resources such as Nutrition, Pastoral Care, Social Work, Resource Library, NYP support groups, Nursing Staff and other Patient Navigators. Coordinates Cancer Support Groups, including outreach and publicity, facilitation, arrangement of guest speakers and tracking attendance. Coordinates with other Patient Navigators to develop and make ongoing revisions to the new patient orientation guide and distributes guide to patients as appropriate. Performs other job related duties as assigned.
