Job Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents while collaborating with Registered Nurses, physicians, and other healthcare professionals to ensure the highest standards of care.
This role supports both long-term care residents and rehabilitation patients working toward recovery goals.
Responsibilities
Administer medications and treatments as prescribed by physicians
Monitor and document residents’ health status, including vital signs
Report changes in resident condition to the supervising RN or physician
Assist with activities of daily living (ADLs) as needed
Collaborate with the rehabilitation team to support recovery plans
Maintain accurate and timely documentation in accordance with facility policies and regulations
Provide emotional support and education to residents and families
Maintain a safe and clean environment while following infection control protocols
Qualifications
Current Licensed Practical Nurse (LPN) license in good standing
Strong clinical skills including medication administration and wound care
Excellent communication and interpersonal skills
Ability to work effectively within a team environment
Compassionate, patient-centered approach to care
BLS/CPR certification required
