Job Summary: TCA Health is seeking a dedicated and compassionate Midlevel Provider (Nurse Practitioner or Physician Assistant) to serve as a Full-Time Mobile Van Provider. This role involves delivering high-quality primary and preventive care to children and adults in underserved communities. The Mobile Van Provider will play a vital role in improving access to healthcare by traveling to various community locations, including schools, shelters, and outreach sites.

Key Responsibilities:

  • Provide comprehensive primary care services, including acute and chronic disease management, wellness exams, immunizations, and preventive screenings for pediatric and adult patients.
  • Perform diagnostic and therapeutic procedures as appropriate within scope of practice.
  • Conduct patient assessments, develop treatment plans, and provide education on health promotion and disease prevention.
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated and holistic patient care.
  • Maintain accurate and timely electronic health records (EHR) documentation.
  • Ensure compliance with all state and federal regulations related to mobile healthcare delivery.
  • Engage in community outreach and patient education efforts to enhance healthcare access and utilization.
  • Assist in the development and implementation of policies and procedures specific to mobile healthcare services.
  • Participate in quality improvement initiatives and performance evaluations to enhance patient care outcomes.

You may need to work in any of the TCA Health clinics if coverage is needed at any time.

Qualifications:

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Illinois.
  • Certification in Family Medicine, Pediatrics, or Adult-Gerontology preferred.
  • Minimum of 2 years of clinical experience in primary care, mobile van, community health, or urgent care settings.
  • Experience working with diverse populations and a commitment to health equity.
  • Ability to work independently in a mobile healthcare setting with limited on-site supervision.
  • Strong interpersonal and communication skills.
  • Proficiency in using electronic health record (EHR) systems.
  • Valid drivers license and ability to travel within the community as needed.

Work Environment: This position operates primarily in a mobile healthcare unit, requiring travel to multiple sites. A flexible schedule may include some evening and weekend shifts based on community needs.
