About Position: The primary responsibility of the MNT Dietitian is to be part of the healthcare team to provide nutrition counseling and education to Piedmont Health Services’ patients. Evaluate the patient’s nutritional status and requirements, evaluate patient readiness and confidence to make changes, and develop and help patients implement nutrition care plans. Communicate nutrition care plans to the physicians and other health professionals. Perform tasks that will make MNT financially stable.

- (Also serving Siler City CHC - 224 South 10th Avenue, Siler City, NC 27344)
- This position is a split position between Moncure CHC and Siler City CHC. The late night typically alternates between the two sites. One week the 8a-8p schedule is at Moncure on Mondays and then the following week at Siler City on Thursdays.
- Schedule: Monday: 8:00 am - 8:00 pm, Tuesday: 8:00 am - 5:00 pm, Wednesday: 8:00 am - 5:00 pm, Thursday: 8:00 am - 5:00 pm, Friday: 8:00 am - 12:00 pm
- Required Travel: As Needed Between Moncure and Siler City CHC)
Duties/ Responsibilities –
- Determine patient’s needs based on the medical and nutrition risk identified. Follow the format set by AND using ADIME.
- Provides individualized counseling in nutrition and physical activity based on needs identified.
- Assess medications related to nutritional health issues and providing prescription recommendations, when appropriate, to the patient’s PCP.
- Scan medical schedule for potential patients that day or for the future.
- Consults with medical providers, social workers, nurses, and other healthcare providers as needed to provide optimal care.
- Conducts counseling in culturally appropriate way to meet the needs of the patients.
- Properly document, based on contact, on all forms (i.e. bariatric forms) and in EHR.
- Based on professional discretion determines follow-up appointments for the patient.
- Contacts other health professionals or agencies if needed to provide optimal care for the patient.
- Works with other health center staff or outside medical professionals to provide care for patients.
- Demonstrates the ability to work with diverse patient populations served.
- Provides thorough assessment of each patient and determines appropriate nutrition diagnosis.
- Based on patient centered approach, determines educational topics.
- Effectively presents education topic with appropriate handouts.
- Performs phone calls to inform patients of labs, help adjust medications, and/or follow-up.
- Documents nutrition education contacts.
- Interprets, evaluates, and utilizes pertinent current research relating to nutrition care.
Qualifications –
Education: Bachelor’s Degree from an accredited institution approved by the ACEND (Accreditation Council for Education in Nutrition and Dietetics) with a major in nutrition and a Registered Dietitian.
Required: Degree and Licensure's
- Ability to teach; analytical thinking
- Ability to provide care to geriatric participants; knowledge of clinical dietetics
- Must be able to work under pressure
- Excellent interpersonal, oral and written communication skills
Preferred: Spanish Speaking; Clinical dietetics experience; Interested in becoming a certified diabetes educator or is a diabetes educator.
Current/valid License: Registered dietitian and licensed dietitian in the state of NC (RD, LDN).
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
Pay Range: $59,050 - $79,358 (commensurate with years of experience)
