This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Spartanburg Area Mental Health Center 250 Dewey Ave, Spartanburg, SC 29303.

Under limited supervision, serves as the Centers Housing Coordinator to ensure patient access to appropriate community-based housing. Complete assessment of patient living in Community Care Residential Facilities (CRCFs). Assess patients psychiatric status to determine the ability to move to a lower level of care in the Housing continuum.

Responsibilities for the Mental Health Professional I/Housing Coordinator:

  • Provide oversight for the Centers housing programs, including Community Housing, Homeshare, Homeshare Respite, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCFs), and other supportive housing programs, as applicable. Coordinate administrative requirements of the agencys housing programs, including payment contracts and patients agreements, outreach and recruitment efforts for Homeshare providers, and completion of assessments for patients living in CFCFs. Assess patients psychiatric status to determine ability to move to a lower level of care in the Housing continuum
  • Coordinate with the agencys Transition Program staff, including Clinical Care Coordinators (CCCs) and Transition Specialists, MCO, CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists. Partner with the local HUD Continuum of Care, public housing authorities, and affordable housing owners and property managers. Coordinate patient housing referrals and move-ins and maintains wait list for each Center housing program.
  • Enter and maintain Community Housing data in the agencys Electronic Health Record. Prepare and submits the monthly Community Housing report.
  • Enter and maintain Center Homeshare program and patient information in the agencys SharePoint site and CIS/Electronic Health Record.

Minimum and Additional Requirements

A masters degree and professional experience in guidance, counseling or the social sciences and two years of post-graduate experience in the clinical counseling program. Must be licensed or eligible for licensure in SC and willing to work towards licensure in SC within the first six months of employment. Employee must be able to meet OMH credentialing requirements.

Additional Requirements:

In order to be credentialed for core services as a Mental Health Professional at the SC Office of Mental Health, applicants must provide evidence of clinical experience providing direct patient care. This experience may be documented as a practicum or internship during graduate program, or post-graduate clinical experience. Preferred Qualifications

Additional Comments

Post hire, employees must:

  • Successfully complete the Basic Fundamentals course with a minimum grade of 75.
  • Be able to pass CPR and Managing Agitation in the Psychiatric Setting (MAPS) Certification.
  • Knowledge of psychopathology and client management within the community -based psychiatric setting.
  • Experience working with Chronically mentally ill adults and seriously emotionally disturbed children and youth.
  • Organizational and communication skills are required for this position.
