Position Summary: The Mental Health Professional (MHP) functions as a clinical member of the multidisciplinary team and provides treatment, rehabilitation, and support services to clients enrolled in the LTSR program. Responsibilities include, but are not limited to, the following: • individual and group therapy • case management • medication monitoring • assistance with the activities of daily living (ADL) • crisis intervention • psychosocial education • vocational support and supportive counseling The incumbent is expected to provide recovery-oriented interventions according to each clients individual treatment plan and maintain professional relationships with other providers, both internal and external to the program.
Minimum Qualifications: Masters level degree or higher in generally recognized clinical discipline and one (1) year of mental health clinical experience. Registered, licensed or certified if profession is governed by such a board in the Commonwealth.
Exceptional communication skills, both written & verbal, organizational skills & the ability to effectively work with a team of other mental health professionals. Strong clinical skills with working knowledge of the mental health system, psychiatric rehabilitation principles and recovery-oriented practices. Ability to effectively work with persons with mental illness, developmental disabilities, substance abuse disorders and forensic issues and encourage their participation with comprehensive treatment plans. Act 33/34/31 and FBI Clearances Valid drivers license with access to a vehicle during scheduled work hours.

Position Summary: This position is responsible for the coordination and implementation of client treatment plans that address the needs and interests of all residents in the RTFA. The incumbent is responsible for assisting in the coordination of referrals, conducting client intakes, gathering all relevant information needed to develop treatment strategies that will help the client in achieving his/her goals, and organizing and coordinating program discharges. Crisis management and resolution are also important functions of this position.

Minimum Qualifications: Scale 1 - High school diploma or equivalent, demonstrated written and verbal skills, knowledge of practical self-care skills, ability to deal effectively with persons with mental illness, current PA drivers license and access to a vehicle during work hours. Scale 2 - Bachelors Degree in a related field; Or Associates degree in a related field with two years full time (or equivalent) related experience; Or High school diploma (or equivalent) with eighteen months full time experience as a Rehabilitation Technician One (Scale 1) within the agency; Or A high school diploma (or equivalent) and three years of related full time (or equivalent) experience from outside the agency. Scale 3 - Bachelors degree in a related field and at least twelve months experience as a full time Rehabilitation Technician One and/or Rehabilitation Technician Two within the agency; Or An Associated degree with at least three years full time direct care experience within the agency; Or A high school diploma with at least 48 months experience as a Rehabilitation Technician One and/or Rehabilitation Technician Two within the agency; or A Bachelors degree in a related field with eighteen months of full time (or equivalent) experience from outside the agency; Or Associates degree in a related field with at least thirty months of related experience from outside the agency.

Position Summary: The Clinical Care Specialist is responsible for the assessment, planning and coordination of services for residents of the Personal Care Home. Duties include the following: • conducting resident assessments • developing and implementing comprehensive support plans • coordinating treatment and rehabilitation services as indicated • providing supportive counseling and skills teaching based on each residents individualized plan The incumbent will co-facilitate multidisciplinary team meetings with the Program Supervisor or Director in order to utilize and incorporate the skills, clinical judgment and input of each member of the team when developing new or modified support plans.
