We are currently seeking a passionate individual for the position of Full-Time Mental Health Housing Specialist. In this impactful role, you will be instrumental in providing support to individuals within our supportive housing program.
POSITION RESPONSIBILITIES
With Administrator, you will assist residents with the development and implementation of a long-range service plan. You will also assist in securing and maintaining entitlements, such as SSI, SSD, PA, Medicaid, SNAP, and Section 8 housing. Conducting weekly home visits and one phone contact with assigned residents, you will provide additional support to residents in crisis as needed and document all contact with collateral service providers.
Your responsibilities will also include assessing and providing referrals to residents regarding any unmet service needs, attending to providing and modeling the necessary development of ADL skills, budgeting, cooking, household maintenance, personal hygiene, and the use of public transportation. You will assist in the transportation of residents to MH programs and events, ensuring good relationships with neighbors and the general community.
Additionally, you will be responsible for reporting all housing maintenance issues through the appropriate procedure, maintaining accurate and current phone logs and resident records. You will immediately notify your supervisor of all potential crisis or emergency situations and document all incidents and occurrences.
You will also conduct resident house meetings as needed, conduct fire drills at each residence bi-monthly, and assist residents at the time of moving in and out. You will meet weekly for supervision, review residents rent monthly with your supervisor, attend and participate in staff meetings and scheduled in-service training, and attend appropriate agency and interagency meetings.
With your supervisor, you will be responsible for discharge planning of residents and act as a task supervisor when interns are assigned to the Supported Housing Program. You will also ensure agency compliance requirements by grants through site visits, data collection, and tracking systems. You will be responsible for rotating on-call duties and other related activities.
POSITION REQUIREMENTS
High School Diploma or Equivalency with experience in human services required. A Bachelors Degree in Human Services or related field is preferred. Experience working with individuals with severe mental illness is also preferred. You must have a valid NYS Drivers License with a safe driving record and access to a personal and reliable vehicle.
PHYSICAL REQUIREMENTS
Sitting, standing, stooping, squating, walking, and moving intermittently during work hours. You may push or pull 10-15 pounds daily and occasionally push or pull up to 50 pounds. You will also drive an agency vehicle.
POSITION RELATIONSHIPS
Reports to Community Services Coordinator. Supervises interns.
TRAINING
Training takes place Monday - Friday, 8:30 am - 4:30 pm.
SHIFT
Monday - Friday, 8:30 am to 4:30 pm, with on-call availability.
PAY RATE
$21.00 per hour.
LOCATION
Main Office in Islip Terrace, NY.
