Responsibilities:

The Director, Memory Support Clinical Practice and Education provides leadership and is an authority in dementia care clinical practice and education. In this role, the Director, Memory Support Clinical Practice - Education functions as an expert practitioner, consultant, educator and coach in dementia care. This individual is responsible to ensure that center teams' throughout the Company are poised for successful clinical dementia care practice, delivery, and education. Works in partnership and collaboration with Director, Memory Support Program Development, Clinical Operations, Nursing Education, Therapeutic Recreation, Rehabilitation, and other disciplines to create a seamless process for education delivery, practice development, and adherence to Company, standards of care, and regulatory practice guidelines to promote safety, quality, and efficiency. The level of support will be based upon assessment and identified needs. Networks across divisions and corporate to bring optimal resources -- remotely -- to meet center support needs.

Assures that programs and education curriculum are evidenced-based, align with industry standards and in compliance with company, regulatory, and legal requirements.


Leadership/Administrative
1. Develops and maintains strong working rapport with interadepartmental personnel within Areas and Regions for collaboration, planning, and problem resolution. Exemplifies and promotes intraprofessional communication, collaboration, and practice.
2. Serves as a model and coach for Area, Regional and Center Leadership Teams.
3. Provides on-site and/or remote consultation and support for Area/Region/Center team members.
4. Serves as an active member of the National Specialties Team Practice Council, other internal committees, as well as external committees to promote the organization.
5. Leads and facilitates committees and work groups, as indicated.

Business Excellence
1. Provides resources and coaches application of compliance standards and evaluation tools to drive quality improvement.
2. Ensures proper tools, education and resources are disseminated to center teams to provide the highest quality Memory Support programming in every center of operation.
3. Serves as a resource for problem solving and supporting Regional and Center Leadership.
4. Promotes the implementation of Centers Quality Assurance Process Improvement (QAPI) to support quality, safety, and efficiency in clinical outcomes.
5. Works with Outcomes Department to facilitate the collection and analysis of outcome measures related to dementia care and programs.
6. Coaches in survey management relative to dementia specific standards, as indicated.
7. Works in partnership to safeguard compliance with regulatory agencies.
8. Maintains community and professional relationships to ensure GHC Memory Support market awareness and promotion.

Qualifications:

1. Bachelor's Degree is a relevant professional field such as nursing, social work, rehabilitation, therapeutic recreation, or related field required. Master's degree in preferred.

2. Licensed in respective discipline.

3. Extensive knowledge of Alzheimer's disease and related dementias including individual and programmatic approaches to care.

4. Five years' experience in providing direct care to individuals with dementia and their families

5. Knowledge and experience in long term care required

6. Advanced skills in computer programs such as Word, Excel, PowerPoint, and Zoom.

7. Must be able to travel.
