Job Summary:

The Memory Care Coordinator oversees the operations of the Memory Care Unit. This position is a working supervisory position that is part of the Nursing Leadership team. This individual will maintain the standards of care to ensure each resident experiences a welcoming and safe environment.

Required Qualifications:

1. Bachelor's Degree in Mental Health professions, Social Services Profession, or Nursing.

2. Licensed LPN or RN preferred. 

3. One (1) year experience with dementia and/or Alzheimer's residents within the past 5 years. 

4. Minimum of twelve (12) hours of dementia-specific training within three months of initial employment and six (6) years of training annually thereafter. 

5. Working Knowledge of nursing service, mental health, rehabilitation, and geriatric nursing preferred. 

Responsibilities/Duties:

  • Demonstrate and model company culture and values. 
  • Oversee the operation of the Memory Care unit. 
  • Ensure personnel assigned to the unit have received required in-service training and assist with training as needed. 
  • Ensure that care provided to residents is consistent with in-service training, current dementia care practices, and regulatory standards. 
  • Maintain a professional working relationship with medical team members of the community, as well as outside providers. 
  • Responsive to the needs of residents, families, and legal representatives in a professional manner as each situation requires. 
  • Ensure each resident is assessed prior to admission to the Memory Care unit, is appropriate for the unit, and meets criteria for admission. 
  • Coordinate and oversee unit activities to ensure appropriate life enrichment is accomplished. 
  • Encourage, coach, and support Memory Care unit team members to inspire compassionate and positive relationships with residents. 
  • Participate in hiring process for team members who will potentially be working in Memory Care
  • Perform other duties as assigned.
