Medical Social Worker (MSW) – Contract Assignment

Shift: Day Shift | 5x8-Hour Schedule
Hours: 8:00 AM – 4:30 PM
Assignment Duration: 13 Weeks

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and experienced Medical Social Worker (MSW) to join a collaborative healthcare team in Napa, California. In this role, you will provide psychosocial support, discharge planning, care coordination, and resource management to patients and families throughout the continuum of care.

The ideal candidate will possess strong clinical judgment, excellent communication skills, and the ability to work effectively within an interdisciplinary healthcare environment.

Key Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement individualized discharge and care coordination plans
• Provide emotional support, counseling, and crisis intervention services
• Coordinate community resources and post-discharge support services
• Collaborate closely with physicians, nurses, case managers, and healthcare teams
• Advocate for patient needs and ensure access to appropriate healthcare resources
• Maintain accurate and timely clinical documentation
• Educate patients and families on available healthcare and community support programs
• Assist with transitions of care and continuity planning

Required Qualifications

• Master’s Degree in Social Work (MSW) required
• Previous hospital or healthcare social work experience preferred
• Strong knowledge of discharge planning and care coordination
• Excellent interpersonal, organizational, and communication skills
• Ability to handle sensitive situations with empathy and professionalism
