Job Title: Master Social Worker
Location: Fairfield, California
Schedule: Monday – Friday, 8:30 AM – 5:00 PM



Position Summary
  • The Master Social Worker provides comprehensive clinical social work services, counseling, discharge planning, and case management for hospital patients and their families. This role focuses on assessing psychosocial, emotional, financial, environmental, and post-discharge needs while collaborating closely with the interdisciplinary healthcare team to develop and implement individualized plans of care.
  • The Social Worker serves as a patient advocate, facilitates access to community resources, supports continuity of care across the healthcare continuum, and promotes optimal patient outcomes through integrated mind-body-spirit care. All activities are performed in accordance with organizational mission, values, policies, procedures, and regulatory requirements.

Essential Duties and Responsibilities
  • Conduct comprehensive psychosocial assessments of patients and families to identify emotional, social, financial, environmental, and discharge planning needs.
  • Develop, implement, monitor, and evaluate individualized psychosocial plans of care.
  • Provide direct clinical social work services, counseling, crisis intervention, and support to patients and families.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary team members to coordinate patient care.
  • Develop and coordinate discharge plans to ensure safe and effective transitions across the continuum of care.
  • Monitor patient progress toward goals of care and identify barriers affecting treatment outcomes or length of stay.
  • Facilitate referrals and linkages to community resources and support services.
  • Advocate for patients and families while promoting patient-centered care.
  • Interpret social, psychological, emotional, financial, and family dynamics for healthcare team members.
  • Participate in quality improvement initiatives, clinical practice teams, community networking, and other organizational activities.
  • Maintain accurate, timely, and complete documentation in patient medical records, departmental systems, MIDAS+, Cerner, and other required platforms.
  • Provide support to patients and families experiencing grief, loss, adjustment challenges, and life transitions.
  • Demonstrate commitment to ongoing professional development and enhancement of clinical skills.
  • Comply with all departmental, hospital, regulatory, and accreditation standards.
  • Support organizational goals focused on delivering exceptional patient, family, and community satisfaction.
  • Perform other duties as assigned.
Education
  • Master of Social Work (MSW) from a school of Social Work accredited by the Council on Social Work Education (CSWE).
Licensure
  • Current Licensed Clinical Social Worker (LCSW) license required.
Experience
  • Experience in acute care, hospital social work, discharge planning, case management, or related healthcare settings preferred.
  • Experience working with diverse patient populations across the lifespan, from neonatal through geriatric populations, preferred.
Physical Requirements
  • Frequent sitting, standing, walking, bending, stooping, and twisting throughout the workday.
  • Regular use of computer systems and keyboarding.
  • Ability to travel between facilities and departments as needed.
  • Reliable transportation available for coverage of referrals and assignments across locations.
