Scheduler – Home HealthFull-Time | San Luis Obispo Office | NOT REMOTE

Central Coast Home Health and Hospice is a locally owned and operated organization with an outstanding reputation throughout the Central Coast. We are currently seeking a full-time Scheduler to join our Home Health team in our San Luis Obispo office.

The Scheduler plays a key role in coordinating and scheduling patient visits for multiple disciplines while ensuring exceptional patient care and communication. This position requires the ability to manage multiple phone lines, respond to patient and staff inquiries efficiently, and maintain organization and professionalism in a fast-paced environment with frequent interruptions. 

Work Schedule: Monday through Friday, 8:00 AM – 5:00 PM Weekend Day rotation required

Responsibilities Include:

  • Coordinate and schedule patient care visits for clinical team members
  • Answer and manage multiple phone lines with accuracy and professionalism
  • Assist patients and staff with scheduling questions and care coordination
  • Ensure patient visits are planned and delivered appropriately
  • Maintain organization and prioritize tasks effectively in a dynamic environment
  • Provide excellent customer service to patients, families, and staff

Qualifications:

  • Current RN or LVN license, or Medical Assistant Certification required
  • Previous healthcare experience preferred
  • Excellent written and verbal communication skills
  • Proficiency with EMR systems and scheduling software.
  • Ability to multitask and work efficiently under pressure
  • Proficient in Microsoft Outlook and Word
