White County Rehab & Nursing is seeking a Medical Records & Transportation Coordinator to join our team in Carmi, IL. This is a full-time, direct hire opportunity in a skilled nursing facility.


Pay

  • $16.00 - $18.00 per hour (non-CNA)
  • $19.50 - $23.22 per hour (CNA rate)

We are open to candidates who are currently CNAs or willing to become CNA certified.


Position Overview

This role is responsible for accurately requesting and maintaining medical records, scheduling resident appointments, and coordinating and providing transportation to and from medical appointments and/or facility-related events.

This is a dual-role position that requires strong organization, attention to detail, and the ability to manage both administrative and resident-facing responsibilities.


Key Responsibilities

Medical Records:

  • Organize, maintain, and update resident medical records
  • Compile documentation including progress notes, care plans, and discharge summaries
  • Ensure records are complete, accurate, and compliant
  • Sort, prep, scan, and review charts
  • Maintain confidentiality and follow release-of-information guidelines

Transportation Coordination:

  • Schedule and coordinate transportation for medical appointments and facility-related outings
  • Provide transportation for residents as needed
  • Communicate schedules, routes, and timing with staff and vendors
  • Maintain transportation logs and documentation
  • Coordinate vehicle servicing and maintenance with the Maintenance team
  • Ensure vehicles are safe, operational, and compliant

Qualifications

  • High school diploma or equivalent required
  • Minimum 1 year of healthcare-related experience
  • Knowledge of medical terminology
  • Strong organizational and computer skills
  • Ability to multitask and communicate effectively
  • CNA certification preferred OR willingness to obtain CNA certification
  • Skilled nursing facility experience a plus
