Compensation 105k- 120k

Position Overview: 

The Medical Provider reports to the Director of Clinical Services for administrative and operational purposes and to the Medical Director for medical supervisory purposes.  The Medical Provider performs tasks primarily in-clinic and on occasion in the field on the mobile medical unit, including physical examinations, ordering lab and other tests, prescribing medications, administering treatments, evaluating for immunizations, and providing referrals. The Medical Provider collaborates with the entire medical team to provide excellence in patient care. Their work results in improved health outcomes, expanded health education, and enhanced trust in ASHwell services. 

 

 

Core Responsibilities 

 

Patient Care 80% 

 

  • Under the clinical supervision of the Medical Director, provide medical care to patients with a primary focus on HIV care and prevention, Hepatitis C treatment, and STD/STI treatment, plus other needed care including chronic disease management, sexual and reproductive health care, and gender affirming services. 

  • Conduct clinical encounters with patients including taking an HPI; completing physical exams; assessing for social determinants of health, documenting medical, social, family history; ordering diagnostic tests and interpreting results; developing differential diagnoses; and establishing and documenting plans of care. 

  • Provide evidence-based care in accordance with national standards and internal practice guidelines. 

  • Use clinical judgment and consult with collaborating physician when necessary. 

  • Ensure appropriate follow-up for ordered care, including referrals and test results. 

  • Provide cross-coverage for the other medical providers when necessary. This includes review of other providers’ lab results, portal messages, and medication refill requests. 

 

General 20%  

 

  • Serve as a clinical resource for a team of Registered Nurses (RNs), Medical Assistants (MAs), Community Health Workers (CHWs), and other staff members. 

  • Maintain complete and accurate patient medical records. 

  • Assist the Director of Clinical Services in implementing and evaluating best practices, protocols, policies and procedures. 

  • Maintain communication with the supervising physician to review patient records and medical care plans on a routine basis. 

  • Participate in team meetings and organization-wide planning sessions. 

  • Assess matters of clinical compliance and provide recommendations for quality improvement. 

  • Respond to messages from patients and care team members in a timely manner. 

  • Liaison with other members of the care team including Nursing, Social Wellness, and Patient Advocacy to optimize patient health outcomes.  

  • Participate in all required agency staff meetings and events. 

  • Maintain up-to-date calendar and work plan. 

  • Utilize agency-wide systems and protocols for all agency business. 

  • Maintain Chain of Support for internal communications.

Required Qualifications:  

  • Graduation from an accredited Nurse Practitioner or Physician Assistant Program  

  • Unrestricted and valid license to practice as a Nurse Practitioner or Physician Assistant in the state of Texas 

  • Current Drug Enforcement Agency (DEA) registration and valid NPI number (or ability to obtain) 

  • Current CPR certification 

  • Working knowledge of HIPAA regulations  

  • Baseline understanding of gender pronouns, sex positive culture, and HIV de-stigmatization and/or willingness to learn 

  • Strong communication and written skills, and a demonstrated ability to work effectively with diverse staff and populations. 

  • Computer proficiency, including strong skills using web-based applications, electronic medical records, and Microsoft office products.

Preferred Qualifications:  

  • Bi-lingual English and Spanish speaking 

  • Sexual and reproductive health experience including expertise in STD diagnosis and treatment, PrEP/PEP, and gender affirming therapies 

  • Understanding and knowledge of harm reduction practices and approaches 

  • Experience in the treatment of HIV and Hepatitis C 

  • 2+ years of experience in working with diverse populations, including, but not limited to, people who hold any one or mix of the following identities and/or circumstances:  low income, unhoused, Black, Indigenous, person of color, lesbian, gay, bisexual, transgender, queer, questioning, intersex, asexual, gender non-conforming, two-spirit, immigrant, disabled, neurodivergent,  recovering, and/or using substances. 

 

   

This is a full-time position that requires a commitment to the mission of our nonprofit organization. The Medical Provider will report directly to the Interim Director of Clinical Services.
