We are hiring a Physician (MD) to serve in a combined role as Medical Director and on-site primary care provider across our Pennsylvania market, delivering general medicine care to geriatric residents in skilled nursing facilities. This is a role for a clinically confident physician who wants leadership development, a strong interdisciplinary team, and the chance to meaningfully reduce unnecessary hospitalizations. You will be the in-facility primary care presence for short-term rehab and long-term care residents while serving as Medical Director — and Telos handles the administrative and operational load so you can focus on patient care and clinical leadership. Medical Director leadership. Function as Medical Director for your assigned facilities, with a fixed monthly stipend that is separate from RVUs, not census-dependent, and always applicable. CMD training, fully funded. Participate in Certified Medical Director (CMD) training at no cost to you. CMD certification is not required to start. Strong NP/PA support. A dedicated NP/PA clinical team handles routine and regulatory visits, keeping your clinical volume manageable and predictable. Focus on care, not overhead. No practice overhead or administrative burden — EHR, telehealth platform, and administrative coordination are all provided. Never working alone. Collaboration with attending physicians, NP/PA clinicians, and interdisciplinary teams is built into how the practice runs, backed by a CMO who personally tours sites (Dr. Mike Renzi). What you'll do Serve as the facility's primary internal medicine physician and on-site provider (SNF). Provide on-site primary care to short-term rehab and long-term care residents. Perform comprehensive assessments and evaluate residents for significant changes in condition. Manage acute and chronic conditions following established clinical protocols. Prescribe medications and direct treatment plans as appropriate. Supervise and support NP/PA clinicians providing routine and regulatory care. Function as Medical Director for assigned skilled nursing facilities. Lead Quality Assurance & Performance Improvement (QAPI) and survey readiness. Participate in a defined, structured after-hours call schedule with provider-performed, billable telehealth. Complete accurate documentation and CPT coding. Collaborate with interdisciplinary teams, facility leadership, and attending physicians. What we're looking for Active, unencumbered MD license in Pennsylvania (or ability to obtain). Current DEA registration. Board certified or board eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine. Strong communication and clinical leadership skills. Ability to work effectively in a fast-paced, semi-autonomous environment. Preferred: experience in a Skilled Nursing or long-term care setting. Interest in post-acute care, leadership, and community-based medicine.
