Medical Director – Post-Acute Care

Consilium360 is conducting a confidential search for a Medical Director on behalf of a well-established 325-bed post-acute care organization located in the greater Great Neck, NY area.This position will serve as a senior clinical authority for the organization, providing strategic oversight of the medical department while partnering closely with executive leadership to drive clinical excellence, quality outcomes, regulatory compliance, and operational performance.
The Medical Director will play a key leadership role across:
• Clinical operations
• Quality and performance improvement initiatives
• Care coordination and case management
• Pharmacy oversight
• Population health management
• Interdisciplinary collaboration and physician engagement
We are seeking candidates who bring strong executive presence, sound clinical judgment, and the ability to lead in a fast-paced, high-accountability healthcare environment. The ideal candidate will possess exceptional communication and organizational skills, a proactive leadership style, and the ability to effectively manage and mentor medical professionals and clinical support staff.
Qualifications
• Board Certification required
• Prior leadership experience within post-acute care, skilled nursing, rehabilitation, long-term care, or related healthcare environments strongly preferred
• Strong understanding of regulatory and quality standards
• Technology proficient with strong documentation and follow-through skills
Compensation & Benefits
• Base salary: $350,000 – $375,000
