TAG MedStaffing is hiring a Medical Case Manager (Temporary) in Columbus, OH for our client, supporting participant engagement, case management, and service coordination within a community-based program. This role works directly with individuals to assess needs, develop action plans, and connect them to resources that support personal and professional growth. The position partners with program and clinical teams to drive positive outcomes in a structured environment.
Responsibilities as the Medical Case Manager (Temporary):
Participant Support: Build relationships with participants to assess needs, set goals, and provide ongoing coaching.
Case Management: Manage an active caseload and track participant progress against service plans.
Resource Coordination: Connect participants with internal programs and external community resources.
Advocacy: Support participants across education, employment, and social service systems.
Documentation: Maintain accurate case notes, service plans, and progress reports in compliance with requirements.
Collaboration: Work with program staff and leadership to support participant outcomes and program goals.
Qualifications for the Medical Case Manager (Temporary):
Education: High school diploma or equivalent required, associates or bachelors degree in a related field preferred.
Experience: Experience in case management, coaching, or social services required.
Industry Knowledge: Familiarity with community resources and referral systems preferred.
Technical Skills: Basic proficiency in documentation systems and data tracking tools.
Skills & Attributes: Strong communication, organization, and ability to manage multiple cases effectively.
