Job Summary

Grant Funded Position

Responsible for the provision and evaluation of services delivered by assigned Medical Case Managers for delivery of competent, quality patient care and support services for assigned areas.

Essential Functions

  • All team members are expected to be knowledgeable and compliant with Prisma Health's purpose:  Inspire health.  Serve with compassion.  Be the difference

  • Manages a defined case load while performing the duties of Team Leader.

  • Provides leadership and mentoring guidance to assigned staff.

  • Serves as Team Leader, coordinating the work of a group of assigned staff in the day-to-day operations of the team.

  • Assists with hiring and training of new medical case managers.

  • Assists with Social Work Intern training and supervision.

  • Oversees operations of MCMs in the absence of the director per request of director.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Master's degree in Social Work, Counseling or health/human services related field of study.

  • Experience - Three (3) years of experience in ambulatory or acute care setting

  • Knowledge and skills to provide clinical and administrative leadership to assigned team.

  • Knowledge of Healthcare Reform, government and third-party reimbursement systems and contracts. 

  • Organizational, time management, problem solving, and critical thinking skills.

  • Understanding of pathways, procedures, and governmental regulations.

  • Knowledge base representative of the skills and training of a MSW Case Manage preferred
