Medical Case Manager - Marion

Essential Job Functions:
- Conducts medication counts daily as required.
- Performs medication administration/observation in accordance to SMA Healthcare policies and procedures.
- Make referrals to SMA programs and community resources.
- Assists clients needing financial assistance for medications and other medical needs.
- Disposes biomedical hazardous waste per SMA procedures.
- Provides administrative support to nursing staff.
- Reports any concerns regarding the health, safety, or well-being of a client to the appropriate authority.
- Serves a positive role model; utilizes approved behavior modification techniques; provides positive reinforcement and support to clients.
- Completes required program documentation in a timely manner.
- Completes reports as required; daily, weekly, monthly, etc. as assigned.
- Documents service activity notes on clients activities, changes in behavior patterns, medications, etc.
- Collects urine specimens; sends to laboratory if required, utilizes infection control procedures.
- Participates in meetings as required, converses with staff on clients progress or lack of progress.
- Documents when client medications are taken or not.
- Performs general office duties; answers phone, communicates with the public, hospitals, internal and external customers, etc. as required.
- Performs general cleaning responsibilities.
- Ensures the Medication room/cabinet is neat, clean and orderly at all times.
- Attends trainings/in-services at all times and keep up to date.
- Ensures compliance with 65-D-30, CARF, SMA performance standards are followed at all times.
- Assists clients with WIC needs as required.
- Oversee clients arranging medical appointments and arranging transportation.
- Performs related work as required/assigned.
Qualification Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Minimum Education and/or Experience: Possession of a high school diploma or equivalent and 6 months of related human services experience or experience working directly with seriously emotionally disturbed children or seriously mentally ill adults.

Knowledge/Skills/and Abilities:
- Some knowledge of human services and the application of basic client care skills.
- Ability to handle verbally and/or physically abusive and aggressive clients and utilize aggression control techniques to ensure the safety and well being of the client, other clients, and employees in the unit.
- Ability to communicate effectively with suicidal, mentally, emotionally, and/or psychologically disturbed clients in person and over the telephone.
- Ability to defuse potentially dangerous situations, and remain calm in an emergency.
Necessary Special Requirements: Possession of a valid Florida drivers license, acceptable driving record, and proof of personal automobile insurance if required to drive an SMA vehicle and/or use a personal vehicle for SMA business. Complete State of Florida mandatory background screening prior to start of employment. Complete SMA required training during the first six (6) months of employment and updated if required.
Physical: Mobility and ability to bend and reach during an 8-12 hour day. Able to lift minimum 10 pounds. Visual and auditory acuity sufficient to evaluate, intervene, treat, and record client health care needs. Fine motor skills for legible and accurate charting, daily correspondence and presentation, either manually or orally. Work endurance ability to work 8-12 hour shifts with a meal break, as possible. Routine 8-12 hour shifts. Hours and days off may vary. Extra hours may be required. Work assignment locations may vary.
