    Position Summary

    Functions as a Disease Management Case Worker to ensure that patients receive necessary medical and specialty visits, labs, education and referrals as deemed necessary by the Medical Providers; updates and maintains the Disease Registry database system.

    Schedule:

    • Flexible, may require some evening and weekends

    Primary Duties and Responsibilities

    • Performs quality assurance of the medical charts for the Disease Management Program.

    • Performs accurate data entry on a daily basis; continually updating and validating information in the disease registry program based on the identified health indicators.

    • Attends all training sessions and meetings to be current on the disease registry system and management protocols.

    • Regularly generates disease management reports, including patient visit summary forms to ensure improvement of established health indicators.

    • Performs patient education counseling including but not limited to: explanation of patient education materials and handouts, disease specific nutritional education, disease specific chronic disease information, issues with medication compliance, exercise, healthy lifestyle habits and other information to improve patient health outcomes.

    • Coordinates resources to help patients achieve optimal management of their disease.

    • Collaborate with other clinic staff to improve patient health management.

    • Perform duties needed to achieve program's objectives.

    • Continually monitoring disease registry system for anomalies and immediately reports problems to Disease Management Supervisor.

    • Contact and follow up with patients to ensure program objectives are met.

    • Provide coverage for other case workers as needed.

    • Actively Participates in the Quality Assurance process including:

    • Chart review and chart review processes

    • Monitoring implementation of quality improvement program components

    • Contacting specialists and outside professionals

    • Follow-up with patients to ensure compliance with established objectives.

    • Other duties assigned as assigned by Clinic Manager or Clinical Service Director.
